Write down that much needed to communicate to the destined individual or organization. There’s no need to stretch the content. Nobody has time to read the information. It’s enough to have short content with the required information.
Stage 5: Person’s Detail
It is vital to mention the name of the sender and the receiver discussed. Without these, it can not be considered as an official formal letter.
What is a Referral Form?
Referral forms are required by the teachers to evaluate students for special education or receive counseling services. They need training and professional development to recognize signs in students who require a referral. Recognition training is essential for discipline referrals associated with counseling. After the teacher completes a referral form, it requires the signature of the staff members to issue such counseling.
How to Make the Social Work Referral Form an Effective One?
Step 1: Appropriate Heading
In your referral form, provide the main heading of the document as “Social Work Referral Form” to the center alignment of the page. You can mention the grade of students for whom the form is meant to address; either for kindergarten, primary or secondary grade students on whom you are going to place the social work duties. Accordingly, the referral forms will be supervised and evaluated.
Step 2: Main Details
Below the heading, add the basic details of the student and teacher. First, mention the student name and grade. Next, include the teacher’s name and the subject she teaches. Usually, if the referral form is prepared by any other teacher other than the class teacher, you need to mention the subjects she teaches the student.
Step 3: Description Based on the Type of Social Work
After the basic details section, add a few descriptions of the referral program. Explain briefly the reason for conducting such a program in schools, whose one of the probable causes may be to monitor the learning process of students so that they can be guided to enhance their basic decision making.
Step 4: Set Standards
The next section is going to set down the various standards based on which the student will be evaluated. Some of them can be, completing work on time, the ability to work with minimal supervision, attendance, punctuality, respectful behavior and so on. You can add more to these standards and judge the student accordingly.
Step 5: Use Rating Scale
There are various ways of rating the student. You can use a Likert scale or a simple rating scale from 1 to 10 and grade based on the extent. Make sure you select the grade which is true to your knowledge. The scaling should be unbiased and neutral so that the student receives the counseling that he requires.
Step 6: Signature
After you have completed rating the student, the final step left is to sign the document by the teacher and the supervising member. The referral form can become official only after the signature of the concerned teacher and the supervisor or evaluator to confirm that they have read and agree with the report.
