Step 1: Decide the Structure

When the required person starts writing the incident statement, he or she needs to start with an overview of the incident. This means you need to describe the incident. This overview does not need to be detailed. Just providing a sentence about the occurrence will also do. You have to keep it as brief as possible.

Step 4: Incident Details

This is perhaps the most important step of the incident statement form. Sections and sub-sections if needed, need to be provided in the form that will be used to contain information about the incident that has occurred. This includes the basic facts and details of the incident such as the date, location and time of the incident, names of the people involved, etc. Separate columns should be provided for each heading.

Step 5: Provide Accurate and Correct Information

There should be allocated space in the form for the person in charge to write about both sides of the story.

Step 7: Include Graphics

It complements the mentioned statistics and offers further information that the receiver can readily understand. That is why you need to provide ample space in the form to include such graphics.

Step 8: Proofread, Validate and Submit

Finally, when you decided upon and selected what elements to include in your form, you can make a rough draft. Since this is an official document, be sure to proofread it for any grammatical or spelling errors. If you are satisfied with the rough draft, get it printed. Some institutions’ protocols require the forms to be validated by the concerned authorities so make sure to do that if that is the protocol of your institution. Once all these formalities are completed, you can fill up the form with the required information and submit it.
